Is your student doing a sport?


You will need to complete a few steps before your student can try out for a sport.

1. aktivate (formerly Register My Athlete) needs to be updated each school year online.

2. Complete a physical form with your doctor. Students only needs one physical, this is good 6-8th grade.

3. Form B-History Form needs to be completed every year.  No doctor signature required.

4. Athletic Release Form completed for each sport in order to pick up your student from games.
